Agapé Home Care’s free in home consultation allows us to gain a greater understanding of your loved one’s individual needs. An Agapé Home Care licensed nurse, known as a case manager, will meet with you and your loved one to identify all the ways we can assist him or her with living independently.

During our in home consultation, the case manager walks through daily living activities and your loved one’s current health care needs. Based on the information gathered, a comprehensive service plan will be developed to identify how Agapé Home Care can help with these activities.
